Click on the appropriate "Buy Now" button that corresponds to the class you are registering for (Junior Puppy Basic / Senior Puppy / Canine Good Citizen).
Refunds/Cancellations:
Class fee includes a $50.00 non-refundable registration charge to reserve your place in class. Pre-registration is required and must be mailed or submitted online with payment in advance.
In the event that you need to cancel once classes have begun, refunds cannot be given, however we are happy to defer your enrollment until a future class date.

By submitting this form you agree to all of the conditions of registration described herein, including the refund and cancellation policies, and acknowledge and agree that your participation in puppy socialization and dog obedience training classes involves some risk and he/she knowingly assumes all risks attendant thereto. The registrant does herewith disclaim and indemnify and hold harmless any and all persons connected with Merit Puppy Training, L.L.C., from any and all loss, damage, sickness, and expense by reason of participating in puppy socialization and dog obedience training classes.
NOTE: Your puppy should have had at least 2 sets of shots in order to attend class, please check with your vet clinic and either mail with this form or bring to class a copy of your dog’s most recent shot record for my files.
